History

Health at Home, a US CareNet company, began offering its in-home healthcare, wellness and companion care services in 1984. In 2013, the company entered into a partnership with US CareNet, then known as CareSouth, to manage 17 Health at Home locations in 11 states, serving over 1,400 clients at the time. US CareNet boasts an almost 40-year history providing in home health and clinical services. The partnership added a higher level of coordinated care with a wealth of technological resources, as well as robust clinical care and educational programs. In 2014, the company exercised its option to increase its commitment to Health at Home, resulting in US CareNet as majority partner.

Today, Health at Home provides care to more than 3,700 clients annually with over 295,000 annual visits performed in 18 locations across the country, and is adding more locations every year. As part of the Health at Home Advantage, the centralized Care Center provided through US CareNet, proactively manages the health care of its patients with outcomes management, transitional care coordination, telehealth monitoring, callback and wellness calls, and 24/7 on-call and after-hours triage, providing cost-saving services to its patients and clients across the country.

Health at Home agencies meet rigorous standards to become certified by both Medicare and the Accreditation Commission for Health Care (ACHC).
